New Patient Appointments

Summit Chiropractic & Wellness Center is a very busy office, and as a result, same day appointments are often not available. In addition, all new patient appointments require a non-refundable $150 deposit that will be applied to your initial exam.

Please provide Dr. Billiris with all prior lab results, x-ray, MRI, CT, and any other prior imaging and office visit reports from prior treatments and or evaluations. Drop off, email, or mail these items prior to your scheduled consultation date.

Deposit Acknowledgment

I understand that a non-refundable $150 deposit will be collected upon scheduling a New Patient Visit. This deposit will be applied toward the cost of my visit and/or treatment sessions, but will not be refunded in the event of a cancellation by the patient. A fee of 3.95% will be added for all credit card payments.
